Willow Rose Rehab & Health Care, a 98-bed skilled care facility in Jerseyville, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the second quarter of 2017.
The report, released on Monday, lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively.
Willow Rose Rehab & Health Care was cited with an “A” violation and fined $25,000 for failing to include a resident’s Advance Directive in their care plan, resulting in the death of a resident for whom cardiopulmonary resuscitation was not being performed. The facility has requested a hearing.
