Rosewood Care Center of Alton, a 180-bed skilled care center in Alton, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Rosewood Care Center of Alton was cited with an A violation in November and fined $25,000.
