2017-265 Recommendation for the approval of a contact purchase order with Bertelsmann Learning, LLC, d/b/a Relias Learning LLC to provide online learning courses for Community Services staff to maintain certifications and receive CEUs. This contract covers the period of October 1, 2017 to September 30, 2018, for a contract total amount not to exceed $5,307.53. Other Professional Service not subject to competitive bidding per 55 ILCS 5/5-1022(a). Vendor selected pursuant to DuPage County Code Section 2-300.4-108 (1) (b). Community Services Block Grant Funded.

HHS-P-0386-17 Recommendation for approval of a contract purchase order to Symbria Rehab, Inc., for the Community Wellness Center staffing and management for the DuPage Care Center, for the period October 1, 2017 through September 30, 2018, for a contract total not to exceed $65,500.00, per renewal option under Proposal P16-146-GV, first optional one year renewal.
C. Change Order -- HHS-P-0311A-16 - Amendment to Resolution HHS-P-0311-16 (County Contract 2097-1 SERV), Issued to Symbria Rehab Incorporated, for Therapy and Consulting Services: Physical, Occupational and Speech Therapy to the residents at the DuPage Care Center, for the period November 1, 2016 through October 31, 2017, to increase encumbrance in the amount of $145,000.00, resulting in an amended contract total not to exceed of $1,165,000.00, an increase of 14.22% (2097- 0001 SERV)
9. Budget Transfers
Budget Transfers -- Community Services - HUD Continuum of Care Grant - To move funds to the personnel lines in order to spend down the grant 100%. The commodities and contractual expenses for the grant were less than anticipated, therefore the grant can cover more of the personnel costs originally charged to the general fund 1000-1750. $8602.00
