Signal Hill School District 181 issued the following announcement on Aug. 21.

ATTENTION PARENTS

****Health paperwork is still required even though we are doing virtual learning. Please turn in as soon as possible. Contact the school nurse with any questions.***

 

Early Childhood/Pre-Kindergarten:

• Physical Exam on Illinois State Form prior to October 15, 2020
• Documented up-to-date Immunizations prior to October 15, 2020

Kindergarten (or first grade if first time in school):

• Physical Exam on Illinois State Form prior to October 15, 2020
• Documented up-to-date Immunizations prior to October 15, 2020
• Eye Exam on Illinois State Form by Oct 15, 2020
• Dental Exam on Illinois State Form by May 15, 2021

Second Grade:

• Dental Exam on Illinois State Form by May 15, 2021

Sixth Grade:

• Physical Exam on Illinois State Form prior to October 15, 2020
• Documented up-to-date Immunizations prior to October 15, 2020
• Dental Exam on Illinois State Form by May 15, 2021

ALL GRADE LEVELS:

• Up-to-date Immunizations and a physical exam on a Illinois State Form. Pre-school, kindergarten, second grade and sixth grade are the only grades required to have new physicals unless the student is participating in sports. Students participating in sports are required to have a new physical exam each school year.

Transfer Student:

• Same as above per grade level within 30 days of registration
• Out of state transfers must have a current equivalent physical within 1 year from enrollment date (can be from previous school) or must get a new physical on Illinois State Form within 30 days
• Student new to the Illinois School System will need an Eye Exam on the Illinois State Form within 30 days from registration
