City of Collinsville issued the following announcement on Mar 30.
The Utility Tax Rebate application period for power bills dated April 2020 - March 2021 is available April 1, 2021 until May 31, 2021.
To be eligible for the rebate you must be ONE of the following:
- A Senior Citizen (65 years or older) Resident of Collinsville,
- A Disabled Resident of Collinsville,
- or A Low Income Resident of Collinsville
- 65-years of age or older on or before 3/31/2021 (no income limit)
- State or Federally issued identification (must show birth date)
- Entire electric and gas billing from April 2020 through March 2021 (name and address must correspond to identification)
- Illinois Person with a Disability Identification Card stating that the claimant is under a Class 2 Disability
- State or Federally issued identification (must show birth date)
- Entire electric and gas billing from April 2020 through March 2021 (name and address must correspond to identification)
- State or Federally issued identification (must show birth date)
- Entire electric and gas billing from April 2020 through March 2021 (name and address must correspond to identification)
- United States income tax return, IRS Notice 703 or an affidavit indicating that no income tax return was filed for the prior year. Income must not exceed limits for household size in the table below.
Applications will be accepted April 1, 2021 to May 31, 2021 and can be submitted in the following ways:
- From Your Home, online at https://ultra.localgov.org. Use this method to submit copies of your bills.
- At City Hall by signing a LOA (Letter of Agency) to access your AmerenIL or Southwestern Electric statements electronically and your state issued ID and submit online via Azavar.
A letter of agency is needed for Azavar to obtain your power bills. You can download and fill out/print the letter of agency and return it with your signature and ID to City Hall.

An e-mail will be sent to you with the status of your rebate and if approved, your check will be issued within 60 days.
Maximum Utility Tax Rebate is $55.
